Cutaneous cancer is now the most common human malignancy and in the UK 11% of that total is due to malignant melanoma of which probably 80% is related to increased exposure to sunlight. Although it is the least common skin cancer malignant melanoma is the most deadly, and in 1999 it killed over 1600 individuals in the UK. In 2004, in the USA, 55 100 new cases were anticipated. The incidence of this disease is increasing more rapidly than any other malignancy and in males there was a fourfold increase in incidence between 1971 and 1997 whilst there was a threefold increase in women. Cutaneous melanoma is arguably the most widely metastasising neoplastic disease and it has a particularly unpredictable pattern of spread. Although systemic treatments for metastatic melanoma are generally disappointing there is evidence that surgical intervention for solitary soft tissue deposits may be advantageous. The presence of metastatic disease also has a crucial impact on outcome and imaging therefore has an important role in both defining indications for surgical or medical interventions as well as predicting eventual outcome. There is continuing debate as to the place of routine imaging in the assessment of patients with malignant melanoma and this presentation illustrates imaging features of metastatic disease and its role is discussed.
